You’ll sign up for Part A and Part B through Social Security. Once enrolled, you’ll manage your benefit plans through Medicare.

Get your Medicare number

To find your Medicare number, sign in to your Social Security account and view your benefit verification letter.

Get a replacement Medicare card

Sign in to your Medicare.gov account to print your card. You can also reach out to Medicare and ask them to mail you a replacement.

To create a Medicare.gov account, you’ll need to know your Medicare number and your Part A coverage start date. You can find both in your benefit verification letter.

Make changes to your personal information

Medicare uses the name, address, phone number, and date of birth on your Social Security record. Update your personal record if you have any changes to report.

Cancel Medicare Part A or B

Fill out Request for Termination of Premium Hospital Insurance of Supplementary Medical Insurance (Form CMS-1763) (PDF) and fax or mail it to your local Social Security office. You can cancel Medicare Part A only if you pay a premium, and you can cancel Medicare Part B at any time.
